There are four Endourological Fellowships that have
been designed as a one year work study program to allow selected
individuals the opportunity to study endourological techniques at
one or two major endourological
centers of his or her choosing. During the time spent
at each institution, it is anticipated that the fellow will observe
many minimally invasive clinical cases and participate in a clinical
or laboratory research project. The object of the fellowships is
to establish endourology at academic centers outside of the United
States of America.
GENERAL APPLICATION AND INSTRUCTIONS
The instructions set forth in this document have
been prepared to help you complete the application form. Please
read them carefully before completing the application. All applications
for a fellowship award are due on or before October 15, 2007. All
questions regarding the fellowship program, instructions or applications
should be directed in writing to:

Selection Procedure:
After the applications have been submitted, they
will be reviewed by the Executive Committee of the Endourological
Society. Awardees will be notified in writing after January 15,
2008. All applicants will be advised of the results and receive
a summary statement of their grant review where applicable.
Eligibility:
Chief residents, junior academic faculty and members
of the Endourological Society with an academic appointment who are
planning to continue their academic career after completing their
fellowship training.
Stipend:
$2000.00 per month for 12 months. All transportation,
board and lodging will be the responsibility of the fellow.
Nondiscrimination:
It is the policy of the Endourological Society to
operate without discrimination as to age, race, religion, sex, handicap,
or national origin in the employment and promotion of staff on the
selection of participants for the Endourological Fellowship.
Publications:
Any publication arising from the work supported
by the Endourological Fellowship shall acknowledge the Endourological
Society and the corporate sponsorship. The acknowledgement should
read as follows:
"This work was supported in part by the Endourological
Society and (the name of the corporate sponsor fellowship)"
Please provide a copy of any resulting publications
to the Endourological Society throughout your fellowship. In addition,
the fellow will be responsible for submitting a presentation of
her or her work to the Journal of Endourology.
Scholarship Duration:
July 1, 2008 through June 30, 2009.
Application:
Application for the fellowship must be made in writing
to the President of the Endourological Society, Arthur D. Smith,
M.D. The potential fellow needs to include in the application the
following data:
a. Curriculum Vitae stipulating: Present position, education,
internship/residency, hospital appointments, publications, memberships
in professional societies, current research projects, hospital appointments,
conference presentations, etc.
b. Letters of recommendation from three urologists who have
practiced with the individual and are familiar with his or her surgical
abilities.
c. A letter from the Chairman of your department or division
of Urology indicating how your fellowship year will impact upon
the development of academic endourology in the applicant's country.
This letter is most important as it entails the essence for a fellowship
program. This letter must state that you have a position in the
department or division of Urology upon your completion of the fellowship.
d. A one to two page outline from the applicant regarding
- the reasons for seeking the fellowship position
- the reason for selecting the one or two places that he/she
plans to visit
- how the fellowship will impact on his or her career
- future plans after the fellowship is completed.
e. A one page description of the research project in which
the fellow will be participating at each of the institutions he
or she will be visiting or alternatively a research proposal indicating
a project that is possible to complete in the time available. If
no project is available, a candidate can be selected provided he
or she has adequate credentials and support from their home institution;
however, preference will be given to those candidates with a defined
project.
f. A letter of support from the head of the Endourological
Program at each institution where the fellow wishes to spend his/her
time. This letter should indicate the plans for the fellow with
regard to clinical activity and research work.
INCOMPLETE APPLICATIONS WILL BE RETURNED TO THE
APPLICANT. PLEASE GO THROUGH THE ABOVE CHECKLIST AND MAKE SURE THAT
EACH OF THE 5 MAJOR CATEGORIES HAS BEEN INCLUDED IN YOUR APPLICATION
PACKET. INCOMPLETE APPLICATIONS WILL NOT BE REVIEWED.
Host institutions will be given an allowance of
$500 per month to help offset research expenses of the visiting
endourology fellow.
