For graduating Endourology Fellows, in order to receive your credentialing certification, you will be required to submit:

1. Submission of a manuscript to the Journal of Endourology essay contest by April 1st of the year of your planned graduation. The paper may be focused either in basic science or clinical research. Once your essay is ready to be submitted, please visit the website, http://mc.manuscriptcentral.com/endourology. You will need to create an account if you do not already have one. Enter the “Author Center” and then select “create a new manuscript.” In the manuscript type section, please select “Essay Contest 2008 – Clinical or Basic Science” from the drop down options. The abstract should be limited to 250 words. Failure to submit this manuscript no later than April 1st will result in failure to graduate. In order to allow for a timely review of your paper, and with regard to the time constraints of the contest reviewers, there are no extensions beyond the April 1, deadline.


2. Each fellowship program must have a minimum of 3 publications in peer reviewed journals with the fellow as first author on at least 2 of these publications during the years of his/her fellowship.


3. Fellow must write and have accepted either an animal protocol or IRB clinical protocol; the fellow should then proceed to complete this protocol or play the lead role in initiating and moving it forward toward completion.


4. Case Log must be submitted via the website. There are no exceptions. You should be entering your surgical cases as they are being conducted at www.endourology.org.  Cases performed during the fellowship should be entered and up to date as of June of year of graduation.
   
   - Also through this program you can enter all of the teaching conferences or courses which you gave during your fellowship (e.g. lectures or laboratory instruction). A list of these didactic sessions is also required for graduation from the fellowship program.


5. A letter of endorsement from the Director of your Fellowship program need to be sent to the Society. Michele Paoli, the Society’s administrative assistant, will update your information on the Case Log Program when this letter has been received by the office.


6. Submit a listing of all publications, book chapters, videotapes that you produced during your fellowship year(s).

The fellow’s binder of all activities during the fellowship as outlined in #3 needs to be submitted to the Endourological Society by May 1 of the planned year of graduation.  Failure to receive this material by the May 1 deadline will result in delay of recipient’s receipt of certification of graduation from the Endourological Society and will result in a warning being issued to the program director.  Repeated late submission will result in the program being placed on probationary status.   Failure to provide these materials by July 1, will result in no certificate for the fellow and probationary status for the program.  There will be no exceptions to this policy.  Only one fellow will graduate and become credentialed per program per year.
