Episode #19: Management of the Distal Ureter and Bladder Cuff During Minimally Invasive Nephroureterectomy
- Minimally invasive techniques to NUx and specifically management of the bladder cuff have evolved over the last decade. Various techniques to managing the ureteral orifice and bladder cuff have been attempted including cystoscopic, laparoscopic and more recently robotic approaches. In this podcast I will review some of the historical approaches and end with my approach to managing the distal ureter and bladder cuff using the daVinci Xi robotic system.
Episode #18 What is the Best Option for Fluid Management During Ureteroscopy?
- In this podcast, Dr. Michelle Jo Semins of the University of Pittsburgh discusses her insights into new developments in the field of stone management. Fluid management dictates your visualization, which coincides with outcomes. More reliable flow leads to better visualization, and better visualization leads to better outcomes and shorter operating time. Therefore, with proper management one can perform procedures more efficiently and effectively. She notes that the best option is a machine that safely regulates precise pressure leading to less dependency on others.
Endourology Sound Bites #17 Laser Physics for Breaking Stones: What the Urologist Should Know
- In this podcast, Dr. Ghani discusses laser physics and technical parameters for ureteroscopic lithotripsy. I cover the basics of the holmium laser system, characteristics of the HoYAG pulse waveform, peak power and peak power density, and implications for stone fragmentation and efficiency when considering pulse duration, pulse energy, pulse frequency, pulse modulation, total power, and fiber to stone working distance. The relationship between material and fluid absorption, and how this affects safety parameters of intrarenal pressure and heat generation are also discussed.
Endourology Sound Bites #16: What is Holding Back Holmium Laser Enucleation of the Prostate (HoLEP)?
- In this episode Dr. Miller will review the evidence in support of HoLEP as the new gold standard surgical treatment for BPH and LUTS. She will also critically examine the factors that have played a role in adoption of the technique both in the US and globally.
